Neuro-Orbital InflammationOrbital inflammation is a common problem in adults and children. Orbital inflammation can be divided into nonspecific, idiopathic (no specific cause known), and other specific diagnoses. Orbital inflammation impacts both neurologists and neuro-ophthalmologists because all of the entities can cause visual dysfunction (decreased vision, abnormal color perception, afferent pupillary defect, and visual field defect) and decreased motility of the eyes. Treatment Initial therapy consists of systemic steroids. Rapid resolution of symptoms helps confirm the diagnosis. However, inflammation associated with other orbital processes, such as metastasis (spread of cancer) or cellulitis (diffuse inflammation of connective tissue), can also improve on systemic steroids. An orbital biopsy may be needed when an incomplete response to steroid occurs to rule out other orbital diseases. Other treatments include: orbital radiation, methotrexate or cyclophosphamide.
